Potential predictive significance of insulin resistance syndrome in ischemic stroke

Insulin resistance syndrome (IRS), the incidence of which has been steadily rising, represents a significant risk factor of cardiovascular disease, including ischemic cerebrovascular stroke. THERAPY MONITORING OF ASTHMA PATIENTS IN THE PHARMACY

The therapy of 16 patients with asthma was monitored within an 8-month period during which the patiens were visiting the same pharmacy. Their medication problems, imperfection in the inhalation technique, and manifestations and reasons of non-compliance were identified in the course of four sessions. The changes in their understanding of the illness and objective changes in their health condition were detected using semistructured interviews. Mistakes in the use of different types of inhalators were minimized, and a training in cleaning the inhalators and in finding out the number of the remaining doses was provided. The aim of rational pharmacotherapy of epilepsy is to maximize effectiveness as well as to minimize side effects. In comparison with old antiepileptic drugs (1st and 2nd generation), the side effects and interactions of new „3rd generation“ antiepileptics are less known. The overview of efficacy and tolerability of new antiepileptic drugs (gabapentin, lamotrigine, topiramate, levetiracetam, occarbazepine, tiagabine, zonisamide) is based on a metanalysis of 1 462 publications. In terms of interactions, the slow degradation of lamotrigine due to a combination with valproate is stressed.

Osteoporosis: goals of treatment

Ageing may be accompanied by osteoporosis which is not an unavoidable consequence of ageing. There are no characteristic clinical symptoms of osteoporosis. However, osteoporosis may result in low-trauma fractures. Vertebral and hip fractures result in untimely death, disability and dependence of afflicted patients. Osteoporosis can be early diagnosed, prevented and treated before fractures occur. It should not be treated before conclusively diagnosed. The pharmacological treatment of osteoporosis should be stratified with respect to the main factors of the 10-year absolute risk of fracture, i.e. bone loss, age and prevalent fracture.
