THE CONSUMPTION OF ANTIMICROBIAL DRUGS AND ANTIMYCOTICS AT THE SURGICAL WARD OF THE HOSPITAL AND POLICLINIC DURING 2000-2004

Objective: Analysis of prescription and consumption of antimicrobials and antimycotics in a surgical ward at the Hospital and Policlinic during the years 2000–2004 in terms of individual groups of antimicrobials and medicines in the frame of these groups and economic indicators, as the share in total consumption and spending on medicinal products in the first quarter of the period 2000–2004.

Methods: Data for calculating defined daily doses (DDD) were obtained by processing of hospital order forms with the help of pharmacy software. This entry referred per 100 days of hospitalization. The basis for price calculation of one DDD of individual groups of antimicrobials was pharmacy selling prices (wholesaler’s purchase price + VAT + pharmacy mark-up).

Results: The largest share in the consumption of antimicrobials had penicillins; we noticed a tendency towards their increased consumption (37.4 % DDD/100 days of hospitalization during 2000, 48.9 % DDD/100 days of hospitalization during 2004). The most frequently used medicine was amoxicillin in combination with potassium clavulanate – on an average 30.3 DDD/100 days of hospitalization.

The most significant parts of expenses were comprised of penicillins (26.5–44.7 %) and cephalosporins (31.0–41.4 %). We have to consider an important group of antibiotics which in small consumption in DDD represents quite a significant part of antimicrobial costs (imipenem, vancomycin). There was a decrease in consumption of antimicrobials from 91.1 DDD/100 days of hospitalization (2000) to 61.8 DDD/100 days of hospitalization (2004). Total financial expenditures of surgical ward on antimicrobials decreased from CZK 611 million (2000) to CZK 292 million (2004).

Conclusions: The rational use of antimicrobials had a positive impact on lowering expenditures on antimicrobials.
